| The Case Management Department is available to assist patients and their families throughout the hospitalization and discharge process. Case managers and social workers help patients and their family members make educated decisions about their care. They provide information on resources to ease transition into the community or into another care facility when the patient is discharged. Among the services we can facilitate are:
- Counseling
- Home Care
- Medical Equipment
- Nursing Home Placement
- Medical Transportation
- Rehabilitation
- Outpatient Services
- Community Referrals
