Montgomery Hospital Medical Center is committed to delivering the highest quality cardiac care. Through our dedication to superior customer service, we bring you the personalized, expert cardiac care you need to successfully manage your heart disease.
Our Cardiac Nurse Navigators work directly with patients throughout their hospital stay and continue follow-up 30 days post discharge.
The Navigators:
- Assess the educational, emotional and social needs of the patient and their family.
- Assist patients by coordinating their care.
- Communicate test results to you quickly so that you can discuss best therapy options with your patient to manage their heart disease risk.
- Facilitate in scheduling follow-up appointments and obtaining prescriptions.
- Provide patient follow-up for up to 30 days after discharge to ensure success.
